Allow for use of a shared Cloudant instance among multiple search services

We have several instances of the SSS deployed. Each application operates on its own Cloudant instance. It would be nice if all instances could use a dedicated repository database in a single instance to reduce the number of Bluemix resources.

@ptitzler you can bind multiple simple search service apps to a single Cloudant service.
alternatively, you can specify the Cloudant instance the app should use with the environment variable:

SSS_CLOUDANT_URL=<cloudant_host_url>

then for each app sharing the same Cloudant instance you set a unique database name for the app to use with the environment variable:

SSS_CLOUDANT_DATABASE=<db_name>

The readme already mentions SSS_CLOUDANT_URL in the section about running the app locally.
