Any issues with form reset?

So I have a relatively simple form for adding images to a model (a "bookmark" in this instance) but the form.reset() method doesn't seem to be doing anything. Does anyone else have this issue or can see what I'm doing wrong?

Also I'm echoing out the form object above the actual form so I can watch what happens
