Set-up skeleton branch with code from ophbot

Rather than leaving @RiazAziz and @tagimoucia-cheryl to do everything themselves, I propose that we just move the code that is relevant from ophbot to here.
This should

1. Prepare libraries
2. Prepare config
3. Prepare SQL
4. Extract and wrangle inpatients
5. Extract and wrangle some exemplar vital signs
6. Merge the two into a single data frame

The follow-up work would then include

• expanding the data frame to a regular monitoring cadence
• adding in other vitals
• adding in labs
• function to calculate NEWS
• function to calculate ICNARC APS
• presentation layer

I think at that point we'd probably need to split the repo and build one for up-tick/down-tick and one for hospital wide monitoring.
