[LoadStoreOpToLLVM] BlockIO code refactoring (#4636)

Moved `rewriteTensorPointerLoad` to `LoadOpToBlockIOConversion` and
`rewriteTensorPointerStore` to `StoreOpToBlockIOConversion`.
