Integrate IOOS.us menubar/footer framework into IOOS Catalog

Placeholder issue. When funding allows, we need to incorporate the IOOS.us shared menubar and footer framework into the IOOS Catalog.

This will entail redesigning the existing CKAN menus to integrate as well as possible with the IOOS.us shared menubar. May be challenging as CKAN already has two menu bars of its own.

When this is addressed, we should also take care of the following issues at the same time:

• Disable Catalog CS-W service and remove links and documentation from Catalog #245

There may be some other UI issues out there as well...
