This code with angular-confirm 1.1.x and i changed "$modalInstance" to "$uibModalInstance","$modal" to "$uibModal" ,it works well.

I'm not familiar enough with AMD to understand what is happening there. I just loaded the latest code on the demo page and it runs fine. If you figure it out please submit a PR
