Not rendering double underscores

Given the following markdown source:

`role__packer__base_spec.rb`
```

I am getting generated Confluence that appears as:
<pre>
role_<i>packer</i>_base_spec.rb
</pre>

Does anyone have any idea what I would need to change in the code to fix this? If so, happy to send in a merge request.
