Investigate: Skipping user instructions

Sometimes, when the user is instructing via a command to use a git worktree for certain operations, opencode "forgets" about it and messes up the local project, making it difficult for the user to continue working on whatever they're working on. We should somehow make sure the LLM gives the user instructions the importance it deserves.
