Refactor installation steps

If we publish the plugin to the plugin repository (#20), the user won't be expected to install and copy it to their plugin path.

There's also a type in the README where Claude is referenced as Cloud in the prerequisites.

It'd be great to find a way to get the MCP server path in the claude code config to work with a relative path.
