Cannot input in jupyterlab/notebook

hello, I downloaded the widget-cookiecutter.zip and tried to create project in my jupyterlab(version 3.0) and jupypter notebook，it will prompted the author name firstly, but I find no where to input the name and cannot get the next prompt.

I installed the ipywidgets based on its document, all of my jupyterlab/notebook and ipython are in the same environment. 

How should I solve this problem?
