Feature request: per-profile github host

I work on both github.com and github enterprise.
now I need to specify both `GH_HOST` environment variable and my work profile to triage.

it would be convenient if I could specify github enterprise hostname inside the profile.

e.g.

``` yaml
host: git.example.com
done:
  ...
```

then

``` console
$ gh triage -p work
```
