Bug Description

The chat titles here are not helping me ONE BIT to realize what I was working on.

Steps to Reproduce

1. Open Kiro
2. Do a long session that gets continued
3. Chat summary is "New session (Continued)" !?!?
4. Do another 20 sessions. Look carefully. Are your sessions named right?

Expected Behavior

Kiro should name sessions in a way that I can remember what that session was about.
