Multifactor Authentication Fails Instantly After Login Attempt

📝 Issue Description

When attempting to log in using the LastPass CLI, I encounter an issue where the multifactor authentication fails immediately. The CLI briefly displays:

Waiting for approval of out-of-band LastPass Authenticator login...

Then, almost instantly (within ~1 second), it switches to:

Multifactor authentication failed!

This happens consistently on every login attempt.

💻 My Environment

• Operating System: macOS 15.0.1
• LastPass CLI Installation Method: Homebrew (brew install lastpass-cli)
• • Version: LastPass CLI v1.6.1.GIT
• Multifactor Authentication Method: LastPass Authenticator (MFA enabled)

🔁 Reproduction Steps

1. Uninstalled and reinstalled LastPass CLI via Homebrew.
2. Cleared all stored credentials and session data:

   lpass logout --force

3. Attempted login via:

   lpass login [my-email]

4. Observed message:

   Waiting for approval of out-of-band LastPass Authenticator login...
   

5. After ~1 second:

   Multifactor authentication failed!
   

✅ Additional Notes

• I’ve confirmed that LastPass Authenticator is set up and working
• I do not receive a push prompt on my device.

I just had the same issue (using lp authenticator app). I deleted the MFA setup and started over. Now things are working.

I also have the same problem. I turned off MFA and could sign in. I turned it on again, logged out, and checked if I could sign in but unfortunately had the same issue. Looks like I'll just have to disable MFA for now :/
