What version of lldb-dap are you using? Note that we only recently added a --version option so for anything but ToT the interesting information is the LLVM release (or Xcode release) you got lldb-dap from.

This could be a dupe of #128260.

I'm using the latest lldb-dap from main, seems #128260 refers to when there is no debug information at all, in my case the module I'm trying to step into has debug information but it was built on another machine, and the source code paths for it don't exist on my machine and I don't have source maps for them.

I submitted a PR to tackle this: #136494
