fix: improve release notes generation and changelog handling

- Fix release notes extraction logic to properly parse changelog sections
- Add debug output to help diagnose release notes generation issues
- Fix changelog update script to prevent duplicate sections
- Improve handling of empty [Unreleased] sections
- Add better error handling and fallback for release notes

This should fix the empty release notes issue that was occurring
in the past two releases.
