Unable to upload LoTW certificate (PHP and database errors)

When I try to upload my LoTW certificate (.p12 file) I am greeted by the following set of errors—15 PHP errors and 1 database error at the end:

Never mind. I figured it out. The .p12 file I was trying to upload didn't, for some reason, contain my callsign. I re-exported it from TQSL and now it uploads fine.
