MC: Declare MCFragment before MCSection

... so that we can restore `MCFragment DummyFragment;` within
`MCSection` (reverting part of
eedc72b).
This is required to remove `allocInitialFragment` from `MCContext`
and postpone initial fragment creation to `changeSection`
to address some design issues.
