Move hardcoded personal stuff to `injectme/`

Currently there are still a few things in `linkme/` that either have my hardcoded username or 1P item or vault ID. Should move these over to `injectme/` so they are fully portable to other users.

Should also create a catalog in the readme of all the required secrets.
