Feedback form

Hi,
i would like to do a feedback form like in the image below plus an additional choice (that must be visible and mandatory when user chooses the 2 lowest levels of feedback) to specify the insatisfaction reasons.

I'm facing with several issues:

1. Image or ImageSet used to emoji doesn't send any data
2. Input.ChoiceSet could be an alternative to emoji but I can't used it to show/hide the insatisfaction reasons due of lack of support trigger

The form is pretty simple but I'm unable to build it with adaptive cards.
Anyone could point me in the right way without using a refresh card?
