Restrict prog info provider and hook provider to be in same driver

Current design allows prog info provider and hook provider to be present in different drivers. It has 2 issues today:

1. Interlocked operation every time a program is invoked - this creates overhead for program invocation.
2. Not possible for the new hook provider to implement program type specific helper functions, as helper functions are per program type and not per hook type.

Proposal is to add a restriction that prog info provider and hook provider have to be present in the same driver. This will help solve the above 2 problems.
