Does this issue occur when all extensions are disabled?: Yes/No
Unsure. I could not find that option in the VS code menu.

I am using (and have used for years) the following shortcut.

It doesn't look like the bug report feature in the Help menu is working: After I click on "Preview on Github", I don't see all the information attached. So I'm copy-pasting it below just in case.
