Value field not shown in nested program requirement after error condition

Steps to Reproduce

1. Navigate to http://mitxonline.odl.local:8013/admin/courses/program/
2. Click "ADD PROGRAM".
3. Specify a value for Title and Readable id.
4. Expand the "Elective Courses" section.
5. Click "Add Requirement".
6. Ensure that the newly added requirement is Type: Course and a course is specified.
7. Click "Add Requirement".
8. Change the Type to "Operator" for the newly added requirement.
9. Click "Add Course".
10. Click "Save".
11. You should receive the following error message: "Operator must have a Title."
12. Change the Operation of the operator created during step 7 to "Minimum # of".

Expected Behavior

A "Value" field is shown for the operator if the Type is "Minimum # of".

Actual Behavior

No "Value" field is shown.
