Readme contains outdated info about xdebug

The readme says xdebug is not installed by default, but since PHP 7.4 it is. See for example moodlehq/moodle-php-apache@4f67fe9. Thus the pecl install xdebug command is not necessary.

I also found that host.docker.internal won't work (at least for Ubuntu) unless the following is added to the webserver service in docker-compose.yml...

      - "host.docker.internal:host-gateway"
