Tidy up Platform and Application's Debug and Exception code

• I think this code could be simplified, ExceptionArgs and ExceptionReport could be united?
• There is crossover between wxWidgets and our Debug code, can we rely more on wx for crash reporting?
• Cull db insert code, we should do db insertions from another tool spawned to read a report file dumped to the file system
