Clean up documentation

The documentation looks bland and is has some bugs.

1. Go back through the doc comments to ensure accuracy of section definitions and titles
2. Expound a little more in the section headings.
3. Add examples where usage is not clear (flex, wcmx/wcmn, ct, ctr).
