Add Alaska, Hawaii, and Puerto Rico as insets for out/render.png

I added data for Hawaii in #91, but it doesn't show up on the map because it's content is outside of the CONUS bounding box.

The same thing will happen for Alaska and Puerto Rico when added.

(Or could just be 3 more smaller map images we put below the big one?)

Alaska and Hawaii were completed, Puerto Rico is still outstanding.
