Update Shelter Protocol docs with latest opcode format

Problem

The latest updates to the opcodes have not been incorporated into the docs.

Solution

Go through all of the opcodes and verify that the docs match the source. Most of them should match, but some might not. Anywhere that there is a mismatch, correct it so that the docs do match the source.
