Document how to use vCard/Lead custom metadata

We need instructions and additional detail on how QR-to-Lead can be customized through its custom metadata. 

1. Add instructions as a section in the README.  
2. Include a screenshot of the custom metadata records.  
3. Explain how/where these mappings are used.
