Update RFC template for a more scalable numbering scheme.

Instead of trying to figure out what the next RFC number is at the time of PR, "new" RFCs would have an 'xxxx' number in the file name (and header).  After the PR is approved, the author will update the RFC number to the next value of "approved" RFCs before merging the PR.
