Setup proper issue/pr templates and prow

Right now we have no way to merge PRs without manually pressing merge, and I have seen some confusion on what the process is for merging in this repo. We should setup `prow` so that we get the utility of blocking PRs unless they have gotten a lgtm and or approval. This can also be up for discussion.
