Clarify that QR code include both client_id and request_uri (#638)
Merging as per consensus on today's WG call * Clarify that QR code include both client_id and request_uri (and add Michael Z to the acknowledgements as he reported this and some other things previously) * Add Andres Olave to acknowledgements
