Update BasicLabTests OCL collection v20 → v22 (#50)

- Added missing standardized CIEL laboratory test concepts
- Adds 5 new concepts including hemoglobin in umbilical cord blood (required for reference ranges work)
- Removes 9 local AMPATH concepts (4293-4303) superseded by CIEL standards
- Net change: 507 → 511 concepts (+4)

New concepts added:
- 1038: Neisseria gonorrhoeae polymerase chain reaction, qualitative
- 159650: Cerebrospinal fluid appearance
- 161153: Partial thromboplastin time
- 163001: Indirect bilirubin in serum or plasma
- 170239: Hemoglobin in umbilical cord blood (mg/mL)

Removed local AMPATH concepts:
- 4293-4300, 4303: Superseded by standardized CIEL equivalents
