* Delete configuration/backend_configuration/ampathforms/ampath_poc_adult_return_visit_form_v1.6_core-demo.json

Remove Ampath HIV form, it's  consistently broken due to using the Angular Form Engine tech.

* Delete translation file

* Delete english translation
