Why are we doing this?

Recent user interviews have highlighted that users often do have feedback about using OS Docs, however, it would be more useful to be able to capture this as they're encountering issues.
It's anticipated that we will be running an initiative to improve the OS Docs in the near future and having clearer feedback on specifically what's frustrating users whilst they're using OS Docs would be helpful as part of improvement works. Suggest a simple feedback form similar to that used in OpenCodelists would help: opensafely-core/opencodelists#2572

How will we know when it's done?

• Users will have a low-friction way to leave qualitative feedback (e.g. via a visible feedback form)

What are we doing?

Design a feedback form - possibly Google Forms?
Add UI elements to surface the form in relevant places across https://docs.opensafely.org/
