Upstream UDN Enhancement

UDN has already been implemented upstream. This is just moving the
enhancement out of OpenShift and into upstream for future modifications
and reference.

I updated to remove all references to OpenShift. Also, some of the
design changed as we implemented UDN, so updated the document to reflect
the latest design.
