Add a Pregnancy Risk task to ANC home visit - CHW

• Title, "Pregnancy risk - optional"
• It should appear after the ANC health facility visit task
• It should be included in every ANC home visit
• It is not required
• Please reference the data dictionary Tab CHW 2 - ANC home visit, rows 50 -54, highlighted in pink

Once completed:

• If {preg_risk} = "Low", it is green. Otherwise yellow
