Confirmation page: ability to choose PDF to download

Right now, we only support the default PDF:

Like for "Test PDF", we should have a choice in the UI:

Right now we represent a template with:

We need to add:

• use-pdf-template
• pdf-template-name

The <attach> element supports children, specifically:

    <control>control-2</control>
</attach>

Where should we indicate the PDF preferences? Options, on <attach>:

• children elements
• attributes
