MCP Forgetting problem

Claude desktop has this problem of Forgetting that it has access to MCP server. I use a modified Filesystem MCP for personal use. And seems to forget even mundane tasks like accessign filesystem unless it is reminded:

I even tried in claude desktop but still the same.
