Align unit tests

The unit tests and doctests have grown over the development of this project in inconsistent ways.
- align usage of the macros and general construction patterns
- add an example for every public struct that shows ways to construct the struct
- move unit tests into doctests where it makes sense
- have at least one unit test or doctest for every public method
