fix: provide explicit global variable names for UMD global mode

This removes a build warning, and also fixes a few previously undiscovered bugs involving elements referencing other elements when running in UMD global mode.

* provide explicit global variable names to rollup
* fix global var resolution for non-filepath moduleIds
* remove first attempt at solution, no longer used
