Plugin-form-builder should have a disclaimer field

In addition to title, fields and emails, it would be useful if a Form in plugin-form-builder also had the field disclaimer or something like that.

The use-case would be to display additional text in the form. E.g. "By submitting the form I agree that you can store my personal data for the purpose of contacting me"
