Overhaul RAG notebook gen-qa-openai.ipynb (#417)

## Problem

Many outdated dependencies in gen-qa-openai notebook

## Solution

- Update deps and update usage for modern python sdk
- Refactor code to define some functions instead of using purely
imperative code. This improves the documentation value of the notebook.
- Tweak descriptive text to give a clearer narrative structure
