Create an issue template

We currently need two issue templates: one for feature requests and another for bug reports.

As inspiration, we can refer to the Kogito issue template.

It's crucial that each issue template includes the following information:

• The extension the user is using (client, server, or mock).

@mcruzdev besides the extension, is there any other kind of information we'd like to have for features and bugs? If not, could we leave the questions provided in the reference forms unchanged? I'm using the ones in the repo provided by ricardo as a reference
