Update instructions

It looks like, if you already have a document that uses this template, to update the template to a newer version you have to run

```
quarto add quarto-journals/elsevier
```

as well, as it says:

```
Elsevier Journal Format   [Update] 0.4.3 -> 0.4.4 (formats)
```

Can we put this in the README?
