Add support for 101-key keyboards on MacOS hosts #94

The current example osx keymap file only covers a subset of a 101-key keyboard, so not all keys work.
I posted a PR a while back, but I'm not sure how github works, and if I needed to do something else to raise it for consideration, so I'm filing this feature request instead.
Unfortunately I found that left and right control, alt, and windows keys all mapped to only the left keys. I guess that's a problem with my server, but I can't do much about that right now.
