1.4.0 - V13 codecompanion Integration

Update to 1.4.0 along with codecompanion v13

With the latest v13 update of codecompanion there are some breaking changes on how we configure tools. Use strategies.chat.tools in codecompanion v13 instead of strategies.chat.agents.tools. user_approval becomes requires_approval
