Custom Form takes over the Forked State select box

Describe the bug
Forks have long names and they can take over the Forked State selectbox.

We need to be able to rename the forks.
We need to show the name of the select box and truncate

To Reproduce
Steps to reproduce the behavior:

1. fork mainnet and fork it again with the fork button

Expected behavior
A clear and concise description of what you expected to happen.
