`riemann-health`: add a report for physical memory usage

The riemann-health tool report memory and swap as 2 distinct and independent values, but it makes more sense to consider the physical memory as a whole for some use cases.

A single physical_memory value could be used instead of memory and swap if desired (same kind of percent value with warning and critical thresholds).
