Missing recommendations in Profiles and add extra_notes to replace "excluded" extensions

I see that there are recommendations in the Profile YAMLs and in the Ratified Profiles docs but they aren't in the generated profiles.

Also, on a somewhat separate but related note, the "excluded" presence is being replaced simply by a new YAML schema called "extra_notes".

Looks like I added the Ruby & template support for putting recommendations in profiles but never added the missing recommendations to the actual profiles (just put some in my MockProfileRelease).
