Fix issues found by Copilot code review of issue #142 and its PR #143.

Made suggested changes. Also tried nested bold/italics/monospace and it doesn't work but I don't care enough to fix it (could be fixed in a future issue if needed).

Actually, asked Copilot to make the changes to support nested and it did a good job and passed my unit test so I'm keeping it.
